Kenya’s Third Ebola Case Lands Inside a Family Quarantine
Kenya’s third Bundibugyo Ebola case is a quarantined relative of the first death, with no licensed vaccine and a vast Congo outbreak still seeding travel.
Kenya confirmed a third Bundibugyo Ebola case on October 16, a 65-year-old woman who had sat in family quarantine since October 6. She is a close relative of the first two patients, and the Ministry of Health says there is still no evidence of community transmission.
All three infections trace to one imported death. Bundibugyo virus has no licensed vaccine, so Nairobi’s defense is a 21-day watch on 130 contacts, while the outbreak that sent the virus has already killed thousands in the Democratic Republic of the Congo.
A Third Case, Still Inside the Same Family
Director-General for Health Dr Patrick Amoth said the woman developed fever, a sore throat and a general feeling of being unwell while already under watch. Samples tested positive at the National Virology Reference Laboratory and the Kenya Medical Research Institute, and she was moved to an infectious disease unit for supportive care.
The second patient, a 39-year-old man in the same family, fell ill on October 13 inside quarantine and was confirmed the next day. The ministry said he remains stable and is responding well. Kenya’s first Ebola case, recorded after a man who had lived in the DRC died in Nairobi on the night of October 5, is still the only death.
THE NAIROBI CLUSTER
| Patient | Link | Confirmed | Status |
|---|---|---|---|
| Kenyan man returning from the DRC | Index case | October 6 (died October 5) | Died; buried under safe protocols |
| 39-year-old man | Close family of the first case | October 14 | Stable on supportive care |
| 65-year-old woman | Close relative of the first and second cases | October 16 | Infectious disease unit, supportive care |
Amoth warned that more cases can still appear among people already being monitored, and that any new illness should be caught the same way. The ministry’s public line is that the rest of her family, and the wider public, were protected because she was not moving through the city when she fell ill.
The Patient Crossed Three Countries Before Nairobi
Cabinet Secretary for Health Aden Duale said the first patient was a Kenyan national who had lived in the DRC for seven years. He had been ill for about a month and was treated in several hospitals there before travelling by road to Kampala, then flying to Nairobi.
He boarded Jambojet flight 8523 and landed at Jomo Kenyatta International Airport at 1:10 p.m. on October 3. He passed routine port-health screening. A relative drove him to Nairobi Hospital, where staff isolated him with fever, chills, severe fatigue, muscle pain, painful swallowing and bleeding under the skin. Both national labs confirmed Bundibugyo virus. He died that Monday night and was buried on October 6 under safe and dignified burial rules.
WHO Director-General Tedros Adhanom Ghebreyesus said the man became ill in the DRC, received treatment in several hospitals, then travelled via Kampala and died in Nairobi. Tedros said WHO was supporting contact tracing in Kenya, the DRC and Uganda, and advised against travel or trade bans on the information then available.
THE PATH INTO NAIROBI
- October 3, 2026: Lands at Jomo Kenyatta International Airport at 1:10 p.m. on Jambojet flight 8523 and is driven to Nairobi Hospital.
- October 5, 2026: Dies overnight after confirmatory tests for Bundibugyo virus.
- October 6, 2026: Kenya reports its first Ebola case ever; the burial is held under infection-control rules; the woman later named as the third case enters quarantine.
- October 13, 2026: The 39-year-old relative falls ill while already under daily monitoring.
- October 14, 2026: The ministry confirms the second case and says tracing is working.
- October 16, 2026: The 65-year-old relative tests positive, 10 days after Kenya reported the first case.
Early lists were smaller. Duale first named 28 contacts, including relatives and health workers, plus 23 passengers and four crew on the same flight. By the third confirmation the ministry’s list had grown to 130 people: family, friends, the staff who treated the first patient, and travellers who shared the cabin.
Kenya’s Only Tool Is a 21-Day Quarantine Clock
By October 16 the ministry had traced 127 of 130 listed contacts, 97.7%, and placed them in quarantine with daily temperature and symptom checks for 21 days, the longest Bundibugyo usually takes to declare itself after exposure. Officials said that share sits above the 85% global benchmark used to break chains of transmission. Three people on the list had not been found.
Ebola is not spread through the air. It moves through direct contact with the blood or body fluids of someone who is sick or has died, or with objects those fluids have touched. People cannot pass the virus before they have symptoms, which is why isolating a relative at the first fever matters more than a citywide lockdown.
WHO Representative to Kenya Dr Neema Rusibamayila Kimambo said being quarantined means the family and the public are better protected, cutting the chance of community transmission. The two later patients are receiving supportive care in designated treatment units, mainly fluids and treatment of symptoms, which WHO says improves the chance of survival when it starts early.
This is the response working as designed.

The louder argument under that notice was not whether the household cluster was real. It was whether public case alerts themselves would hurt travel, trade and ordinary business, a reflex Kenyans still carry from 2020, even as WHO says Kenya remains open with no curbs on travel, trade or gatherings.
THE WATCH ON 130 CONTACTS
- Traced: 127 of 130 listed contacts are in quarantine, 97.7% of the list.
- Still missing: Three contacts have not been located.
- The clock: Daily checks run 21 days from last possible exposure.
- The bar: Officials cite 85% tracing as the share needed to break transmission.
Duale said that from the May alert through the first death, more than 652,000 travellers had been screened and 267 samples tested in Nairobi, Kisumu and mobile labs at Busia and Lwakhakha. He put trained staff at 4,971. After the first death he named five isolation sites: Kenyatta National Hospital, Moi Teaching and Referral Hospital, the National Police Service Hospital, Port Reitz Hospital and Nairobi Hospital, whose isolation wing has 133 beds, including 45 high-dependency and intensive-care beds. For the third case the ministry said protective gear and screening kit had gone out to 28 high-risk counties and points of entry, with tighter checks on travellers from 10 high-risk countries. The public information line is 719.
Why Bundibugyo Still Has No Licensed Vaccine?
WHO’s Kenya note is blunt: there is no licensed vaccine or specific treatment for Bundibugyo Ebola yet, even though trials are under way in the DRC. Ervebo, the shot used against Zaire ebolavirus, is not licensed for this species, and advisers say evidence that it cross-protects is limited and inconclusive.
That gap is the whole Kenyan problem in miniature. Ring vaccination helped later Zaire outbreaks. Nairobi cannot offer contacts a licensed Bundibugyo dose, so the country is running a classic isolation play against a virus whose early signs look like malaria: fever, tiredness, muscle or joint pain, headache and sore throat. Bleeding, when it comes, usually comes late.
Work on a shot is finally moving, years after the virus was first found in western Uganda in 2007. The University of Oxford’s ChAdOx1 BDBV candidate, built on the same viral-vector platform as the Oxford-AstraZeneca Covid vaccine, entered a UK Phase I study in July. In Uganda, the first participants were vaccinated in Masaka on October 6, the same day Kenya reported its first death.
SHOTS IN TRIAL, NONE IN THE CLINIC
- Ervebo: Licensed for Zaire ebolavirus; WHO vaccine advisers said a DRC trial to test it against Bundibugyo should start before the end of October.
- Oxford ChAdOx1 BDBV: Phase I in the United Kingdom and Uganda, funded by the Coalition for Epidemic Preparedness Innovations; the Serum Institute of India has stockpiled about 620,000 investigational doses.
- Moderna mRNA candidate: Phase Ia work in Canada, with ethical approval on October 7 for Phase Ib in Uganda.
- IAVI rVSV candidate: Flagged by WHO experts as a single-dose option that still needs months before it is ready for an efficacy trial.
Professor Eugene Ruzagira, who leads the Uganda study at the MRC/UVRI and LSHTM Uganda Research Unit, said Bundibugyo virus disease is a form of Ebola with no approved vaccine or specific treatment, and that the Masaka trial is meant to show whether the Oxford shot is safe and can raise an immune response. CEPI’s Dr Nicole Lurie, executive director of emergency preparedness and response, put the DRC crisis in harder words.
The Bundibugyo ebolavirus outbreak raging in the DRC is now the fastest-growing Ebola epidemic in history, and the deadliest Ebola outbreak the nation has ever experienced.

Anthony Scott, chair of WHO’s Strategic Advisory Group of Experts on Immunization, said the DRC’s Tumaini trial should also, in time, fold in the Oxford and Moderna Bundibugyo-specific candidates. That does not help a contact sitting in a Kenyan quarantine room this month.
The DRC Outbreak Is Already the Second-Largest on Record
The U.S. Centers for Disease Control and Prevention calls the DRC epidemic the second-largest Ebola outbreak on record, behind the 2014-2016 West Africa Zaire epidemic of 28,646 cases and 11,323 deaths. It is the 17th Ebola outbreak in the DRC. CDC says it passed 1,000 confirmed cases in 40 days, against about 235 days for the 2018 DRC outbreak to cross the same mark.
The European Centre for Disease Prevention and Control, using Congolese figures through October 14, counted 9,277 confirmed cases and 4,478 deaths. That is a case fatality ratio of 48.3%. Some 924 patients were in isolation, 2,486 people had recovered, and 78.0% of identified contacts in the affected provinces were under follow-up. On October 14 alone the count rose by 65 confirmed cases and 37 deaths.
Historically Bundibugyo killed a smaller share of patients than Zaire virus. A 2024 pooled analysis put the species case fatality ratio at 32.8%. CDC’s chronology lists 149 cases and 37 deaths in Uganda in 2007, and 36 cases and 13 deaths in the DRC in 2012. The 2026 DRC wave is both larger and deadlier than that older record, and it is no longer a two-province problem.
CONFIRMED DRC CASES BY PROVINCE
| Province | Confirmed cases | Deaths | Health zones reporting |
|---|---|---|---|
| Ituri | 6,806 | 3,148 | 28 of 36 |
| North Kivu | 1,991 | 1,133 | 17 of 34 |
| Haut-Uele | 398 | 167 | 7 of 13 |
| Tshopo | 64 | 24 | 7 of 23 |
| Bas-Uele | 13 | 4 | 4 of 11 |
| South Kivu | 3 | 1 | 1 of 34 |
| Sud Ubangi | 2 | 1 | 1 of 16 |
Those seven provinces cover 65 of 167 health zones. Ituri remains the centre, with 6,806 of the 9,277 confirmed cases. WHO declared a public health emergency of international concern on May 17. Uganda recorded 20 confirmed cases and 2 deaths before that outbreak was declared over on August 25. France reported one imported case. A Médecins Sans Frontières staff member who tested positive on September 29 was flown to the Netherlands on October 1.
The $700 Million Kinshasa Cannot Trace
The same week Kenya named its third patient, Africa CDC tried to explain where the response money went. In a communique dated October 15, following a Kinshasa press conference the day before, Director-General Dr Jean Kaseya said international donors had disbursed nearly $962 million straight to international organisations and NGOs running Ebola work, not to the Congolese government.
At a meeting called by the National Institute of Public Health, with Africa CDC’s country director Dr Hyppolite Kalambay, only 30% of the organisations took part and shared figures. Those groups said they had received about $220 million, used $120 million, and still held $100 million. On the $962 million donors say they paid out, Africa CDC says more than $700 million remain unknown to DRC national authorities.
Kaseya told interviewers it was too early to talk of embezzlement. He called the hole a transparency and reporting failure that leaves the government without a full picture of what was raised, what was spent, and what results it bought. Africa CDC said it would back administrative steps up to suspending or withdrawing an organisation’s right to work in the DRC, and that it could share names of groups that refuse to account with other African governments.
Kaseya put the coordination problem in one question: how can a country run an outbreak response without knowing what resources are available, how they are being spent, and what results they are delivering? The Kenyan cluster does not prove that missing paperwork caused the Nairobi import. It does show what an unvaccinated strain does when a sick traveller can still cross two borders and board a domestic-brand flight into a regional hub.
Three Contacts Remain Outside the Net
Kenya’s official position on October 16 was that risk to the public stayed low. WHO had already trained local teams to recognise the disease and handed over about $70,000 in protective equipment, testing kits and infrared thermometers. Kimambo praised the Ministry of Health, the Kenya National Public Health Institute, and county teams for catching the two later cases because those people were already being checked every day.
WHAT WE KNOW
- The cluster: Three confirmed Kenyan cases, one death, all linked to the October 3 arrival.
- The tracing: 127 of 130 contacts are in 21-day quarantine; the second and third patients were already inside it when they fell ill.
- The strain: Bundibugyo virus, with no licensed vaccine and no licensed specific drug.
WHAT IS UNCONFIRMED
- Three names: The remaining contacts on Kenya’s list have not been publicly identified or located.
- North Ubangi: Health officials have described two confirmed cases in a mining area of the Wapinda health zone, which would make it the eighth affected DRC province; Congolese authorities have not issued a matching official declaration.
- Later Kenyan cases: The ministry says further illness among people still being monitored is possible until those 21-day clocks run out.
The 65-year-old woman is in an isolation ward because a relative came home sick and the household was locked down in time. That is a narrower victory than it sounds. Three people who shared air, a car, a ward or a cabin with the first patient are still unaccounted for, and 127 others have not yet finished the watch that is, for now, the only licensed tool Kenya has.
